BEFORE YOU REGISTER A BIRTH

Birth Registration Requirements & Eligibility

Start with the correct registration route. Davao City’s requirements differ for births inside or outside a medical facility, delayed registrations, and births occurring in another locality.

First check for an existing record: A PSA certificate that cannot be found does not necessarily mean the birth was never registered. Verify the local record before starting a delayed registration. Document visuals below explain the requirements; they are not official form images.

01 · TIMELY REGISTRATION / INSIDE A MEDICAL FACILITY

For Babies Born in a Hospital or Clinic

Eligibility: Births of Filipino citizens occurring in Davao City, filed within 30 days. The reporting party may be the hospital, maternity/lying-in clinic, birth attendant, parent, guardian, or another responsible person with knowledge of the birth.

Visual document checklist for timely birth registration inside a hospital or maternity clinic

Documents to Prepare

  • Certificate of Live Birth (COLB), Form 102: Four completed originals prepared through the medical facility.
  • Parents’ Marriage Certificate: One photocopy if the parents are married.
  • Foreign parent’s passport: One photocopy if one or both parents are foreign nationals.
  • Notarized Admission of Paternity: Three originals and one photocopy when a non-marital child is acknowledged by the father.
  • Affidavit to Use the Surname of the Father (AUSF): Three originals and one photocopy where the father’s surname is to be used under the applicable rules.

Prepare paternity and AUSF documents only when they apply. Ask CCRO for the correct execution and notarization requirements.

02 · TIMELY REGISTRATION / OUTSIDE A MEDICAL FACILITY

For Home Births and Other Non-Facility Births

When a baby is born outside a hospital or clinic in Davao City, the CCRO requires additional proof about the birth and the person reporting it. This is still a timely registration if filed within 30 days.

Visual checklist for non-facility birth registration showing informant identification and barangay proof

Documents to Prepare

  • COLB Form 102: Four originals prepared by CCRO personnel and signed by the attendant at birth.
  • Informant’s valid IDs: At least two valid IDs; the Charter calls for two photocopies.
  • Barangay proof: Barangay ID with photograph and signature and/or barangay certification issued to the informant; one original and one photocopy where specified.
  • Parents’ Marriage Certificate: Two photocopies if the parents are married.
  • Parent’s passport: Two photocopies if a parent is foreign; paternity/AUSF affidavits may also be required for qualifying non-marital cases.

Bring complete information about the date, time, place of birth, parents and attendant. CCRO checks the declaration against supporting records.

03 · DELAYED REGISTRATION / BIRTH IN DAVAO CITY

For Births Not Registered Within 30 Days

Eligibility: A person born in Davao City whose birth was not registered in the city’s Register of Births within the 30-day period. The Charter has different requirements depending on the registrant’s age, parents’ nationality and marital circumstances.

Visual checklist for delayed birth registration including Certificate of Live Birth and PSA Negative Result

Core Documents and Supporting Evidence

  • COLB: Four accomplished and signed copies for the applicable Filipino-parent case, including the delayed-registration affidavit on its reverse.
  • PSA Negative Certification / Negative Result: One original plus two photocopies under the 2026 Charter. The city’s public birth-registration guide also specifies six-month validity.
  • Proof of birth facts: Baptismal, school, medical, barangay or other accepted records. The exact number and type depend on your case; the Charter specifically requires two supporting records in certain non-marital cases.
  • Affidavits: A notarized affidavit of two disinterested persons, and a statement about the mother’s whereabouts, may apply to particular non-marital cases.
  • Marriage and identity documents: Parents’ marriage record if applicable; ID or Cedula, plus other evidence requested after CCRO evaluation.

Delayed registration includes a mandatory posting process. Do not treat the PSA negative search itself as proof that no local registration exists.

04 · OUT-OF-TOWN DELAYED REPORTING

For People Born Outside Davao City

Davao CCRO can receive qualifying out-of-town delayed-registration documents to forward them to the civil registrar where the birth actually occurred. The birth is not registered as a Davao City birth. The Charter identifies parents/guardians for a marital child and both parents for a non-marital child as eligible reporting parties.

Visual guide to out-of-town birth reporting including required affidavits and forwarding to the birth locality

Documents to Prepare

  • PSA Negative Result / Certification of Birth: One original and two photocopies.
  • Two documentary evidence records: Examples include baptismal/dedication, school, SSS, voter, PhilHealth, passport or applicable immunization records. Prepare originals and photocopies as directed.
  • Notarized OOT reporting affidavit: State why the birth was not registered; it must be attested by two witnesses.
  • Affidavit of two disinterested persons: Listed for registrants one year old or above.
  • Identity and civil-status proof: Valid IDs, Cedula, marriage certificates and AUSF where applicable.
  • Postal Money Order: Addressed to the civil registrar of the actual place of birth, under the Charter’s OOT checklist.

The receiving registrar in the place of birth may request additional information and will determine the final registration outcome.

CONDITIONAL REQUIREMENTS

Special Situations That Need Extra Documents

Not everyone needs every affidavit or identification record. These are important exceptions to check before gathering originals, arranging notarization or visiting CCRO.

Adults Registering Their Own Birth

The 2026 Charter requires people aged 18 and above to apply for their own delayed registration. A marriage certificate may also be required if the registrant is married. The public CCRO guide requires personal appearance for registrants aged 18 and above.

Non-Marital Child / Father’s Surname

Where the father acknowledges the child or the surname of the father will be used, the applicable Admission of Paternity and/or AUSF may be required. Filing by someone other than the mother can trigger additional sworn statements and supporting records.

One or Both Parents Are Foreigners

Prepare the passport copies and other identity documents specified for your route. Delayed cases involving a foreign parent have an additional checklist covering parents’ birth and marriage records and relevant notarized declarations.

Applicant Filing for Another Person

Where a representative is allowed, authorization or a Special Power of Attorney, the owner’s and requester’s IDs, and an explanation for nonappearance may be necessary. Confirm the applicable right to file before sending a representative.

Missing or Inconsistent Supporting Records

Ask CCRO which alternative baptismal, school, employment, medical, government or barangay records it will accept. Do not guess the birthplace, parents’ names or dates to make documents match.

Additional City-Published Checklist Items

The city’s public birth-registration requirements also mention 2×2 photos, National ID, barangay residency certificates, parents’ records and Cedula in applicable delayed cases. These should be confirmed against the receiving clerk’s current checklist.

Final Document Check Before Visiting

  • Choose timely, delayed or out-of-town reporting first.
  • Verify the birth’s correct city or municipality.
  • Check whether a local birth registration already exists.
  • Bring the right number of signed originals and photocopies.
  • Review exact name spellings, dates and parental details.
  • Obtain conditional affidavits only when applicable.
  • Bring applicant/parent IDs and any necessary authorization.
  • Confirm the correct CCRO receiving office before travelling.

DAVAO CITY · BIRTH REGISTRATION WALKTHROUGH

How to Register a Birth in Davao City — Step by Step

Follow the route that matches the actual birthplace and registration status. Timely, delayed and out-of-town reporting have different forms, review stages and release arrangements.

Before you proceed: This is an office-based procedural guide, not an online application form. The HD images are original explanatory guide illustrations, not screenshots from a live government system. Complete only the steps relevant to your registration type.

01

Before You File

Check the birth location, registration status and records before choosing a route.

Guide to choosing timely, delayed or out-of-town registration View full size ↗
STEP 01

Confirm Where the Birth Occurred

Check the child’s actual city or municipality of birth. Davao City CCRO registers eligible births occurring within its jurisdiction. For a birth elsewhere, confirm the appropriate registrar and out-of-town reporting option.

Do not choose a Davao City birth entry merely because your family lives there.
STEP 02

Check Whether the Birth Is Already Registered

Ask about an existing local record before initiating delayed registration. A missing PSA certificate can mean the registered local record has not yet reached PSA. If a local entry exists, ask about verification or endorsement first.

STEP 03

Select the Correct Filing Route

Within 30 days: timely registration. More than 30 days with no local record for a Davao City birth: delayed registration. Born in another locality: ask whether out-of-town delayed reporting applies.

02

Timely Birth Registration

For births occurring in Davao City and reported within 30 days. Medical-facility and home births follow different preparation rules.

STEP 04

Secure the Accomplished COLB Form 102

For a hospital or maternity clinic birth, ask the facility to prepare four originals of the Certificate of Live Birth. For a home or non-facility birth, CCRO personnel assist with the form and the attendant-at-birth signs it.

STEP 05

Bring Any Conditional Family Documents

Prepare the parents’ marriage certificate if married, relevant foreign-parent passport copies, and any applicable paternity acknowledgment or AUSF documents. For a home birth, bring the informant IDs and barangay proof.

STEP 06

Review Names, Dates and Parent Details

Compare the COLB with supporting records. Check the child’s full name, date and place of birth, mother’s details, parent information and signatures before the packet is submitted.

Request corrections before the form is registered if you notice an error.

Checklist for submitting timely birth registration documents View full size ↗
STEP 07

Submit the Completed Packet for Evaluation

Present the COLB and required documents at the CCRO Birth Registration Division or through the responsible medical facility’s submission process. The receiving clerk checks completeness and correctness.

There is no confirmed universal online birth-registration submission portal in this guide.
STEP 08

Answer the Clerk’s Questions, If Required

For a birth outside a medical facility, the informant provides birth facts during the interview. Receiving personnel prepare or review the entries, obtain required signatures and process the record for official approval.

STEP 09

Receive the Registered Local COLB Copy

After acceptance, the office enters the birth in the register, assigns a registry number, arranges the authorized signature and releases the registered COLB copy under its procedure.

A local registered record is not the same as a PSA-issued birth certificate.

03

Delayed Birth Registration

For a birth that occurred in Davao City but was not entered in the local register within the 30-day filing period.

Guide for verifying a missing local birth record View full size ↗
STEP 10

Verify the Missing Record and Obtain PSA Certification

Ask CCRO to check its local birth register. Obtain the required PSA Negative Result and confirm its validity and the registrant’s identity before filing.

A PSA negative search alone does not confirm that the local birth record is missing.
STEP 11

Organize Supporting Documents by Birth Facts

Prepare the appropriate COLB, affidavits and reliable records establishing name, date, place of birth and parentage. Evidence may include baptismal, school, medical and other permitted records.

Adults aged 18 or older are generally expected to apply personally.
Checklist of delayed birth registration evidence View full size ↗
STEP 12

Complete the Request Slip for Record Verification

At the CCRO front desk, fill out the request slip and submit it for local record searching. The office verifies the information and records the result before the delayed application proceeds.

STEP 13

Present the Delayed Registration Packet

Submit the verified request slip, COLB and all applicable supporting records for evaluation. The receiving officer may ask about inconsistent information, missing proof or conditional affidavits.

STEP 14

Review Required Sworn Statements

Confirm that the delayed-registration affidavit is correctly executed. Complete any applicable paternity, AUSF, parentage or representative authorization documents. Not every applicant needs every affidavit.

Delayed birth registration posting and claim process View full size ↗
STEP 15

Allow the Required Public Posting Period

After CCRO accepts the application, delayed registration involves mandatory posting for ten days and additional official processing. Keep your claim details and follow the instructions for the return date.

Staff-processing minutes do not replace the mandatory posting period.
STEP 16

Return for the Official Registration Outcome

Return with the claim stub or other instructions issued by CCRO. If the application has completed the required review and is approved, follow the office’s registered birth record release procedure.

04

Out-of-Town Delayed Reporting

For qualifying late reporting in Davao City of a birth that actually occurred in another Philippine locality.

STEP 17

Identify the Registrar for the Actual Place of Birth

Confirm the true place of birth and determine the city or municipality responsible for registering the event. Davao CCRO is a receiving and forwarding office for an eligible out-of-town case.

STEP 18

Collect the Out-of-Town Evidence

Prepare the PSA Negative Result, supporting records, IDs, civil-status documents and required notarized affidavits. Confirm the exact requirements for minors, non-marital children and adult registrants.

STEP 19

Prepare the Out-of-Town Reporting Affidavit

The applicant’s notarized statement must explain why the birth was not registered and be attested by two witnesses. For a registrant aged one year or older, an affidavit of two disinterested persons is also listed in the official checklist.

Use the correct birthplace in every affidavit and supporting record.
Out-of-town birth reporting document checklist View full size ↗
STEP 20

Confirm Mailing and Fee Instructions

The published out-of-town checklist refers to a postal money order addressed to the registrar of the actual birth locality. Ask CCRO for current instructions and the receiving registrar’s applicable amount before arranging payment.

STEP 21

Submit the Packet and Answer Interview Questions

Present the documents to Davao CCRO for examination and interview. Staff review completeness and prepare the package for forwarding under the official procedure.

Guide to forwarding documents to the birth locality View full size ↗
STEP 22

Follow Up With the Originating Registrar

Davao CCRO forwards the papers to the responsible city or municipal registrar. Registration is completed there if accepted. Follow that office’s guidance on status, final release and later PSA availability.

Forwarded documents are not proof of completed registration.
17 / 27 minutes Published timely registration staff-processing estimates: medical facility / outside facility.
10 days + processing Delayed registration involves mandatory posting and additional review.
Receiving ≠ registered Out-of-town documents are forwarded to the actual birthplace registrar.

After Registration: Verify and Keep Your Documents

When the responsible registrar releases a local registered Certificate of Live Birth, check the recorded name, date, birthplace, parent details and annotations. Keep the official copy and claim details. If a PSA-issued birth certificate is required, use an authorized PSA channel once the record becomes available.

  • Record not found at PSA: Ask whether the local copy needs transmission or endorsement.
  • Incorrect detail: Consult the registrar about the proper correction process.
  • Delayed or forwarded case: Follow the claim instructions from the responsible registrar.

KNOW THE DIFFERENCE

Local Registered COLB vs. PSA Birth Certificate

Once the birth is accepted and registered, the local registrar can issue the appropriate local record under its procedures. A PSA-issued birth certificate is a separate request, dependent on the record’s availability in the PSA system. Do not assume that a PSA copy is immediately available after local registration.

AFTER REGISTRATION · BIRTH RECORDS GUIDE

After Birth Registration: Local COLB, PSA Copies & Electronic Endorsement

Once the birth is registered, the next step depends on the document you need. Learn how to check your local record, request a PSA-issued certificate and resolve a record that cannot yet be found at PSA.

Read this first: Filing a birth and ordering its PSA certificate are different transactions. The illustrations here are explanatory process guides, not captures of an official government website or actual issued certificates. No specific PSA availability date is guaranteed.

Local COLB Registered and maintained by the responsible local civil registrar.
PSA Certificate Issued through authorized PSA request channels when the record is available.
Electronic Endorsement Used for a locally registered record not found at PSA. The endorsement service is free.
01

Verify Your Registered Local Birth Record

After official acceptance, start with the record held by the relevant city or municipal registrar.

Illustration showing which Certificate of Live Birth fields to check after registration View full size ↗
STEP 01

Check Names, Dates and Registry Details

Review the issued local registered Certificate of Live Birth (COLB), where available. Check the child’s complete name, date and actual place of birth, parents’ names and any important annotations.

Keep the registration number or other reference given by the registrar. If you find an incorrect entry, ask the office about the proper correction procedure instead of altering the document.

Check the record carefully before using its details for a PSA certificate request.
STEP 02

Keep the Issued Local Copy and Claim Information

Retain the genuine local record, official receipt if one was issued, and any claim instructions. If the office has not yet released the record, follow the date and requirements it provided. Do not assume every birth registration produces an instant downloadable certificate.

STEP 03

Confirm Whether You Need a Local Copy or a PSA Copy

A Certified True Copy from the Local Civil Registrar is different from a PSA-issued birth certificate. Schools, embassies, employers and other receiving institutions can have different document requirements.

Ask the receiving institution which copy and whether an annotation or other certification is required. Davao City CCRO lists both local copy and authenticated PSA copy services, but availability and request procedures must be confirmed with the office.

Illustrated comparison between a local civil registry copy and a PSA-issued certificate View full size ↗
02

Request a PSA Birth Certificate

Use an authorized channel and enter details that match the registered birth record.

STEP 04

Allow for Record Transmission and Availability

A new local registration may not yet appear in the PSA Civil Registry System. The time until the record is available can vary. Avoid assuming it must appear after a fixed number of days; ask the relevant registrar or PSA channel how to check.

Visual guide comparing PSA online requests, PSA outlet appointments, and authorized partner channels View full size ↗
STEP 05

Choose an Authorized PSA Request Channel

PSA identifies PSA Serbilis and PSAHelpline as authorized online services. You can also check the applicable PSA CRS outlet appointment system or an accredited Batch Request System (BREQS) partner.

Confirm the current document fee, requirements and delivery or collection rules before paying. Never enter personal information on an unofficial copycat site.

For a direct official starting point, visit the PSA’s authorized online-channel information .
STEP 06

Prepare the Details Needed for the Request

Use the child’s full name, date and place of birth, and the parents’ information as recorded. Follow the selected PSA service’s current identification, requester eligibility and authorization requirements. Check spelling before submitting your order.

STEP 07

Track the Request and Check the Issued Certificate

Keep the reference provided by the authorized service. Follow its release or delivery instructions and check the issued document for accuracy. If the result shows no matching record, use the decision guide below before starting any new registration.

03

If PSA Says “No Record Found”

The crucial question is whether the birth is already registered at the correct local civil registrar.

STEP 08

Do Not Automatically File a Second Birth Registration

A negative result from a PSA search does not always mean that no local registration exists. Incorrect search details, record transmission or other processing issues can affect the result.

Keep the genuine result or order reference and ask the correct Local Civil Registrar to check its register.

Decision guide explaining what to do when PSA cannot find the birth record View full size ↗
STEP 09

Ask the Correct Local Registrar to Verify the Record

Bring the recorded birth details and any available local COLB copy. The registrar for the actual place of birth can verify whether the event was registered and explain the appropriate next step. For births occurring elsewhere, contact that locality’s registrar rather than assuming Davao holds the record.

If the Local Birth Record Exists

Ask the registrar about electronic endorsement to PSA, record transmission or other appropriate verification. You generally do not need to create a duplicate birth registration.

If No Local Birth Record Exists

Ask whether delayed birth registration applies. This involves a separate requirements and evidence process, not electronic endorsement of an existing record.

04

Electronic Endorsement and Follow-Up

For a birth that is already registered locally but cannot yet be found in PSA’s central system.

STEP 10

Request the Proper LCRO Endorsement Procedure

Explain that PSA could not locate the record and provide the details requested by the responsible Local Civil Registrar. The registrar verifies the existing entry and determines whether electronic endorsement is appropriate.

Four-stage illustration showing local verification, electronic endorsement to PSA, processing and later certificate request View full size ↗
STEP 11

The Local Registrar Endorses the Record to PSA

PSA describes Electronic Endorsement as a service in which the responsible Local Civil Registry Office transmits eligible locally registered civil records for processing in the PSA CRS system.

The electronic endorsement service itself is free of charge. This does not necessarily waive any separate charge for requesting a PSA-issued certificate or other unrelated documents.

The applicant should work through the responsible local registrar, not upload a birth record to a made-up endorsement portal.
STEP 12

Check for Updated PSA Availability

Ask the registrar how to follow up and whether a status or reference is available. Processing times are case-dependent; do not rely on an unverified guaranteed release date. Wait for the applicable PSA or registrar instructions.

STEP 13

Request the PSA Certificate Again When Appropriate

Once the record is available for issuance, use a PSA-authorized channel. Keep the request reference, follow current identification and payment instructions, and check the final certificate against the local entry.

Local entry comes first

If a local birth record exists, verify it before pursuing duplicate registration.

Endorsement is free

The PSA electronic endorsement service is free; PSA document-order fees can be separate.

No guaranteed PSA date

Record availability and exceptional review times differ by situation.

Common Record-Release Situations

These checks can help you choose the right office without paying twice or filing an unnecessary second registration.

My local COLB has a spelling error.

Ask the relevant registrar about evaluation and the proper correction route. A PSA certificate order will not automatically fix an incorrect local entry.

The birth happened outside Davao City.

Ask the civil registrar of the actual place of birth about the registered record and endorsement. An out-of-town filing does not make Davao the birth locality.

The PSA request has no matching record.

Keep the negative result and verify the local register. Use endorsement only where an existing local record qualifies.

An institution needs an annotated record.

Confirm the exact document and annotation required. An ordinary certificate or local copy may not meet a particular institution’s requirement.

BIRTH REGISTRATION HELP & TROUBLESHOOTING

Common Birth Registration Problems & Solutions

Missing birth records, rejected documents, delayed applications and incorrect entries can make registration confusing. Find out why these issues happen and what practical action to take before contacting the City Civil Registrar’s Office.

Explore Common Issues

10 SOLUTIONS
01 PSA RECORD NOT FOUND PSA cannot find my birth certificate, but I was already registered.

A birth can be recorded in the local civil register without immediately appearing in PSA’s central records.

What to do

  • Contact the registrar where the birth actually occurred.
  • Ask staff to verify the existing local Certificate of Live Birth.
  • If the record exists locally, inquire about electronic endorsement or the appropriate transmission procedure.
  • Follow the registrar’s instructions before requesting another PSA copy.

Important: PSA confirms electronic endorsement is free of charge. Separate certificate-order fees may still apply.

02 UNREGISTERED BIRTH My birth was never registered and I do not have a birth certificate.

If the birth was not entered in the appropriate local register within the required period, delayed registration may apply.

What to do

  • Confirm the actual city or municipality of birth.
  • Ask the appropriate registrar to check for an existing record.
  • Obtain the required PSA Negative Certification.
  • Collect birth evidence, supporting records and applicable affidavits.
  • Submit for evaluation under the correct delayed-registration route.

A late application may require a mandatory posting period. Registration is not automatically approved upon submission.

03 EXPIRED PSA NEGATIVE RESULT My PSA Negative Result is old. Can I still use it?

Davao City’s public delayed-birth registration checklist specifies six months of validity from issuance for the PSA Negative Result.

What to do

  • Check the issue date on the certification.
  • If older than the stated period, ask whether you need an updated negative certification.
  • Confirm that the searched name, birth date and birthplace are accurate.

Before ordering another copy, confirm the current receiving office’s requirements to avoid unnecessary spending.

04 INSUFFICIENT BIRTH EVIDENCE I do not have enough documents to support delayed registration.

Delayed registration requires evidence establishing the registrant’s identity and circumstances of birth.

Possible supporting records

  • Baptismal or dedication certificate.
  • School records or Form 137.
  • Relevant medical or immunization records.
  • Applicable SSS, PhilHealth, voter or passport records.
  • Barangay certification where accepted.

Davao City’s checklist calls for at least two accepted documentary records in the applicable delayed-registration cases, alongside other required evidence.

Do not invent records. Ask CCRO which available alternative documents it will accept after evaluation.

05 INCORRECT PERSONAL DETAILS My name, date of birth or parents’ names do not match my supporting records.

Conflicting entries can prevent the registrar from determining the correct birth information.

What to do

  • Compare all relevant supporting documents.
  • Check the actual local registry entry, if one exists.
  • Identify whether the problem is a new-form error or an error in an existing registered record.
  • Ask CCRO whether a revised form, further proof or a formal correction petition applies.

Important: Existing registered records cannot simply be changed by handwriting over an entry. Some corrections require administrative or judicial procedures.

06 OUT-OF-TOWN BIRTH I live in Davao City but was born in another province. Can I register here?

The actual place of birth determines the registrar responsible for registration.

Davao CCRO handles qualifying out-of-town delayed registration reporting for births that happened elsewhere.

What to do

  • Confirm the city or municipality of birth.
  • Prepare the out-of-town reporting affidavit and supporting documents.
  • Ask CCRO about eligibility, forwarding arrangements and the destination registrar’s fees.
  • Follow up with the registrar where the birth actually occurred.

Remember: Receiving an out-of-town packet does not mean Davao City registers the birth as a Davao birth.

07 HOME BIRTH REQUIREMENTS The child was born at home and we do not have a hospital birth record.

Births outside a medical facility have their own registration requirements. A hospital-issued document is not the only possible starting point.

What to do

  • Contact the CCRO Birth Registration Division.
  • Prepare accurate information about the birth and attendant.
  • Bring the informant’s valid identification.
  • Obtain the required barangay evidence.
  • Follow CCRO instructions for preparing and signing the Certificate of Live Birth.

If the 30-day filing window has already passed, ask whether delayed registration requirements now apply.

08 PARENTS & PATERNITY DOCUMENTS The registrar is asking for paternity documents or an AUSF affidavit.

Additional requirements can apply when the child’s parents are not married and the father acknowledges the child or use of the father’s surname is requested.

What to do

  • Clarify whether Admission of Paternity or an AUSF is actually required for your circumstances.
  • Ask about the correct legal form, signatories and notarization.
  • Prepare parents’ IDs and other supporting records requested by CCRO.
  • Verify how the intended surname should legally appear in the record.

Do not assume: An AUSF or paternity affidavit is mandatory for every birth. Applicability depends on the child’s legal circumstances.

09 ADULT REGISTRANT I am already 18 or older. Can a relative register my birth without me?

Davao City’s public delayed birth registration requirements specify personal appearance for registrants aged 18 and above.

What to do

  • Prepare your own IDs and relevant supporting birth evidence.
  • Check the required personal-appearance arrangements.
  • Ask which additional documents apply if you are married.
  • If you cannot appear, contact CCRO directly and ask whether any lawful alternative is available.

Do not rely on an assumed representative authorization. The receiving office must confirm the applicable rules.

10 PENDING REGISTRATION / RELEASE I submitted everything, but my registered birth record is still not ready.

Processing depends on the type of application. A complete timely registration is different from a delayed or out-of-town case.

What to check

  • Confirm that the receiving office has accepted all required documents.
  • Check whether a mandatory posting period is still ongoing.
  • Keep any claim stub, reference and genuine payment receipt.
  • For out-of-town cases, ask whether documents have been forwarded to the birthplace registrar.
  • If already registered locally, ask about the proper local copy or later PSA certificate request.

Important: A submitted application, a completed posting period and a PSA-issued certificate are different milestones.

Do Not Start a Second Birth Registration Without Verification

If an application is pending or the birth certificate is missing from PSA, first establish whether the birth already exists in the local civil registry. Electronic endorsement of a registered record and delayed registration of an unregistered birth are different processes. The proper route depends on what the responsible registrar finds.

Prepare These Details Before Asking for Help

Having clear information helps the office identify the problem and explain the right requirements. Share personal records only with authorized government personnel.

Parents’ correct details as supported by reliable records.

Existing local birth record or available copy, if one was issued.

Relevant supporting evidence such as baptismal, school or medical records.

Based on published Davao City Civil Registrar’s Office birth-registration requirements and Philippine Statistics Authority electronic endorsement guidance. Requirements are subject to official evaluation. DavaoCityPortal.org is an independent information website.

BIRTH REGISTRATION · QUICK ANSWERS

Birth Registration Frequently Asked Questions

Get clear answers about when to register a birth, the documents required, delayed and out-of-town applications, parental details and PSA-issued certificates in Davao City.

01 REGISTRATION DEADLINE When should a newborn’s birth be registered in Davao City?

A birth occurring in Davao City should normally be registered within 30 days from the date of birth to qualify as timely registration.

For hospital or clinic births, coordinate with the medical facility about preparation and submission of the Certificate of Live Birth (COLB).

For home or non-facility births, contact the City Civil Registrar’s Office (CCRO) about the appropriate reporting procedure and supporting proof.

Tip: Do not wait until the last day. Missing signatures or documents can delay the submission.

02 HOSPITAL & HOME BIRTHS Are birth registration requirements different for hospital and home births?

Yes. Hospital and maternity-clinic births generally use a COLB prepared through the medical facility.

For births outside a medical facility, the registrar may require additional information about the attendant and informant, valid identification, barangay evidence and the properly completed COLB.

Documents related to the parents’ marital status, foreign nationality or acknowledgment of paternity may also apply in either route.

Check the complete Birth Registration Requirements before visiting the office.

03 DELAYED BIRTH REGISTRATION Can I register a birth after 30 days, including if I am already an adult?

Yes. If the birth occurred in Davao City and no existing registration is found, the delayed birth registration procedure may apply.

Supporting documents may include the applicable COLB, a PSA Negative Result, affidavits and records establishing the registrant’s name, date and place of birth, and parents’ details.

Davao City’s public checklist specifies personal appearance for applicants aged 18 and above. It also lists additional documents depending on the circumstances.

Delayed registration involves official evaluation and a required posting procedure; submission alone is not approval.

04 OUT-OF-TOWN BIRTH REPORTING Can I file in Davao City if I was born in another province?

Davao CCRO accepts qualifying out-of-town delayed birth reporting applications.

This allows eligible documents to be received in Davao City and forwarded to the civil registrar responsible for the actual place of birth.

The out-of-town procedure may require a PSA Negative Result, supporting records, affidavits and applicable forwarding arrangements.

Remember: The birth is registered by the appropriate birthplace registrar, not as a Davao City birth merely because the applicant currently resides there.

05 ACKNOWLEDGMENT & SURNAME Can a child use the father’s surname if the parents are not married?

In qualifying circumstances, RA 9255 allows a child born to unmarried parents to use the father’s surname when the legal requirements for paternal acknowledgment and surname use are met.

An acknowledgment of paternity and an Affidavit to Use the Surname of the Father (AUSF) may be needed, depending on the child’s registered details and the circumstances.

The applicable documents should be registered or processed through the proper civil registrar. Ask CCRO which forms and signatories apply.

An AUSF is not mandatory for every birth registration. Do not prepare or pay for conditional documents unless they apply to your case.

06 LOCAL RECORDS & PSA Why is my birth certificate not yet available from PSA after local registration?

Local birth registration and issuance of a PSA-certified birth certificate are separate government processes.

A birth record may already exist at the Local Civil Registrar but not yet be available through PSA’s central records.

First, verify the record with the registrar for the actual place of birth. If it exists locally, ask whether an electronic endorsement or another record-transmission procedure applies.

If there is no local registration, a delayed-registration procedure may instead be required.

Important: Electronic endorsement of an eligible existing local record is separate from ordering a PSA certificate. The endorsement service itself is free.
