Death Registration Requirements & Eligibility
The documents depend on where the death happened and whether it is being registered on time. Use the guide below to identify the correct route, prepare the required originals and copies, and avoid making unnecessary trips to different government offices.
Notify the responsible health officer after a death, as applicable under reporting rules.
Ordinary deadline to register a death with the civil registrar of the place of death.
A death occurring in Davao City is normally registered through its City Civil Registrar.
When the Death Occurred in a Hospital
The hospital or attending medical professional normally prepares the Certificate of Death and certifies the medical cause of death. A family representative should check that the document is complete before it proceeds through health-officer review and civil registration.
Prepare these documents
- Accomplished Certificate of Death — 4 copies Obtain the completed certificate from the hospital. Check the deceased’s name, date and place of death, and certified cause of death.
- COVID test result or Certificate of No Swab — 1 original + 2 photocopies This remains listed in Davao City’s published 2026 checklist. Confirm with the receiving office which document it currently requires.
- Hospital DOA certification — 1 original + 2 photocopies, only if applicable Needed when the person was declared dead on arrival (DOA), rather than dying during admission.
Before filing: Ask the hospital or funeral service about any embalmer signature and the City Health Office approval required for the completed Certificate of Death.
When the Death Occurred at Home or Elsewhere
For a death without medical attendance, the nearest relative or a person with knowledge of the death should promptly report it to the health officer. The City Health Office determines the appropriate medical certification procedure before the death can be registered.
Documents for an outside-hospital death
- Accomplished Certificate of Death — 4 copies Coordinate with the CCRO and City Health Office for the correct form, medical review and signatures.
- Barangay Certification — 1 original + 2 photocopies Required in the published checklist for a death occurring at home.
- COVID test result or Certificate of No Swab — 1 original + 2 photocopies Listed in the 2026 Charter; confirm the currently accepted evidence with City Health or CCRO.
- Police Report — 1 original + 2 photocopies, if dead on the spot/scene Applies to relevant scene-of-death circumstances; ask police and health authorities before arranging registration.
- Autopsy Report — 1 original + 2 photocopies, if applicable For cases referred for medico-legal examination. It is not a routine requirement for every home death.
Requirements for Delayed Death Registration
If more than 30 days have passed and the death is not recorded in Davao City’s Register of Deaths, the case may qualify for delayed registration. The registrar checks whether a record already exists before evaluating affidavits, supporting evidence and the proposed Certificate of Death.
Core delayed-registration records
- PSA Negative Certification of Death — 1 original + 2 photocopies Shows that the central PSA search did not locate a record under the searched details.
- Notarized Joint Affidavit of Death — 2 originals + 1 photocopy Executed by two disinterested persons; include the required valid IDs showing their dates of birth.
- Evidence relating to the death and burial Cemetery keeper’s certification (1 original + 1 photocopy), and a printed photograph of the tomb or lapida (1 original print + 1 photocopy), as listed in the Davao checklist. Ask which alternative evidence applies if burial was different.
- Informant’s Cedula and ID Two photocopies of the community tax certificate (Cedula) and two photocopies of one valid informant ID.
- Proof of relationship when relevant Birth or marriage records, or other documents identifying the informant’s relationship to the deceased, if required for evaluation.
- Death at home Barangay Certification: 1 original and 1 photocopy.
- Death in a hospital Unregistered Certificate of Death: 3 or 4 original copies.
- Accident or other external cause / dead on arrival Police Report or hospital DOA certification, as applicable: 1 original and 1 photocopy each.
Time matters: Delayed death registration can involve a mandatory public notice, verification and additional official assessment. Do not assume same-day registration.
Special Cases That Need Different Records
Some situations require additional proof or different health and civil registry procedures. The correct route depends on the actual facts, not simply on whether a family member has a Certificate of Death.
Dead on Arrival (DOA) vs. Dead on the Spot (DOS)
A DOA case may require a hospital-issued certification; death at a scene may require a Police Report. They are different supporting documents, not interchangeable labels.
Accidents, unexplained deaths or medico-legal examinations
Coordinate with the police, health officer and medico-legal authority as appropriate. A report or autopsy may be required before medical certification and registration can proceed.
Fetal death or death shortly after birth
The 2026 Charter lists special Certificate of Death and, in certain circumstances, Certificate of Live Birth combinations. Ask the hospital and CCRO to identify the correct records based on gestational age and whether there were signs of live birth.
Filipino Muslim or Indigenous Peoples cases
Davao City lists a separate documentary route that includes a Certificate of Death and barangay certification. Confirm the currently required health certification and reporting arrangements with CCRO.
Death outside Davao City or outside the Philippines
Ordinary registration belongs to the civil registrar where the death occurred, not automatically where the family lives. Overseas deaths may require Philippine consular reporting. Exceptional cases such as a death in transit have separate jurisdiction rules.
Before You Visit the Civil Registrar
Check that the Certificate of Death is complete, and confirm whether the Health Officer’s approval or funeral service’s signature is still needed. Keep genuine originals safe and organize photocopies by the correct registration route.
- Correct full name and date/place of death.
- Medical cause of death recorded by the authorized professional.
- Required signatures, originals and photocopies.
- Barangay, hospital or police proof only when applicable.
- Valid informant ID and supporting relationship records for delayed cases.
- PSA Negative Certification and affidavits if filing late.

How to Register a Death in Davao City
Follow these six steps for a death being registered within 30 days. The hospital, funeral service, City Health Office and Civil Registrar may each have a role.
Prepare the Death Certificate
For a hospital death, obtain the completed Certificate of Death from the hospital. For a home death, contact City Health and secure the applicable barangay documents.
Get the Required Signatures
Check the deceased’s name, place and date of death, and medical certification. Coordinate with the funeral service for the embalmer’s signature where required.
Visit the City Health Office
Submit the certificate for the health officer’s review and approval. Follow the office’s instructions for any applicable health-related payment or supporting report.
Submit Documents to CCRO
Bring the approved certificate and supporting records to the Death Registration Division of the Davao City Civil Registrar’s Office for examination.
Complete Review and Payment
Correct any missing details identified by the clerk. If fees or a burial permit are required, pay only through the authorized government counter and keep the Official Receipt.
Receive the Registered Record
Follow the clerk’s release instructions and check the registered death record for errors. Keep the local copy safe; a PSA-issued certificate is requested separately.
What if registration is late?
After 30 days, ask CCRO about delayed registration. You may need a PSA Negative Certification, affidavits and evidence of the death. Do not file another record without checking the local register.
